1 Mimeo 4H-89 October 1952 Purdue University Agricultural Extension Service Club Member's Achievement Report Book Suggestions All reports should be submitted to the County Extension Office on the date requested by the county. Selected records are duo from the County Extension Agents to the State 4-H Club Office by September 15. Each member should be encouraged to use his or her own initiative to plan and prepare the report. The report should give a clear, concise story of the club member's own achievements. All materials included in the book and the story should have a direct relation to the club member's accomplishments. Ribbons, medals, souvenirs and similar materials should not be included. Be sure the front cover bears the club member's name, address, county, and contest entered. Use standard size cover or manila folder and 8 1/2” X 11" paper. The following suggestions may be used: 1. Cover (use plain manila folder, cloth or paper notebook cover). Use notebook rings or paper fasteners (not clips) to hold the entire report securely in the cover. 2. Use good quality plain white paper 8 1/2" by 11". Use one side of the sheet only, 3. Index of contents (including page numbers). 4. Picture of the member with his or her name, age, address, and county printed or written below. 5. "Standard Report Form for 4-H Club Members", This is an important part of the report and should be filled out as completely as possible. 6. The pictures and clippings should be grouped by years or projects. (a) Include photographs or newspaper pictures relating to the member's own project or those of his or her club, if a junior leader. The caption under each picture hould tell how the picture relates to the project. Give the year the picture was taken or clipped from the paper. (b) Include newspaper clippings relating to the member's own 4-H club activities or those of his or her club group. Underline name and statement relating to member's own work. 7. Include 4-H Record Books of current year. Place 4-H record bopks in large envelope and fasten inside the back cover of the completed report book. Write the names of the project books on the envelope. The achievement record submitted is representing the club members of the county. It should have the material well presented so that the member's accomplishments are easy to find. The story gives further opportunity to tell about the materials presented in the record and also add other interesting things that cannot be recorded on the blank.
